ELITE ART OF PHLEBOTOMY TRAINING reserves the right to cancel the class due to circumstances beyond our control such as weather, disasters, etc. or if minimum registration is not met. You will be scheduled for the next class or you may receive a full refund. High school diploma, or GED is required to take the Phlebotomy Workshop. We do not provide job placement, promise or guarantee jobs. We specialize in Phlebotomy, CPR, workshops that provide continuing education units and a certificate of completion. Registration is mandatory. If the student requires any medical attention while attending the class/workshop, the student is responsible for all expenses incurred. ELITE ART OF PHLEBOTOMY TRAINING is not responsible for or liable for any expenses. You are not registered for the class until payment is made. I do agree to participate in the live draws (allowing individuals to practice on me and will not hold ELITE ART OF PHLEBOTOMY TRAINING liable for any injury that I may sustain from having blood drawn by fellow participants. While every measure will be taken to ensure that the venipuncture technique is being performed correctly and safely under the supervision of a Certified Instructor we are not responsible for any injuries do to practice on the artificial arm or you volunteering to have your blood drawn.
